Job summary

Interim HealthCare of the Twin Cities seeks a Home Care Nursing Supervisor, RN to oversee nursing staff delivering care to patients and clients. You will collaborate with the Director of Nursing to implement processes that improve patient outcomes and service quality.

The role requires MN RN licensure, 2+ years as an RN with at least 1 year in supervision, travel within the service area, CPR, and strong leadership. Includes a $5,000 signing bonus and comprehensive benefits.

Qualifications

• Graduate of an accredited school of nursing and licensed to practice as an RN in the State of Minnesota.
• Minimum two (2) years' experience as an RN, preferably in home care, and at least one (1) year supervisory experience.
• Able to travel within a geographic area serviced by the office. Reliable transportation, valid driver's license, and proof of auto insurance.
• CPR Certification

Responsibilities

• Supervises, supports, motivates, and develops professional and paraprofessional employees within the nursing department.
• Assists with recruitment, including the development of recruitment strategies, interviewing, and hiring of qualified staff.
• Collaborates with the Homecare Director of Nursing and the Director of Education to ensure onboarding, orientation, and ongoing education of the employees within the nursing department.
• Participates in performance management for assigned staff, including evaluations, pay increases, coaching, and performance improvement plans.
• Manages time and attendance, time cards, sick days/PTO, and staff scheduling to maintain adequate staffing.
• Plans, assigns, and monitors work to ensure maximum productivity and high-quality patient care.
• Reviews staff documentation for accuracy and timeliness; resolves employee concerns.
• Uses clinical decision making to guide the nursing team and serve as a resource to the care team.
• Identifies Medicare appropriate needs for skilled home care and hospice.
• Leads/participates in meetings with leaders and staff as needed.
• Interacts with referrals, patients, families to ensure exceptional service and resolve issues.
• Participates in QAPI and quality improvement initiatives.
