Job summary

Enhabit Home Health & Hospice seeks an experienced nurse to oversee coordination of home health services, ensuring care aligns with standards and company procedures. The role focuses on collaboration with physicians, referral sources, patients, caregivers, and staff to deliver seamless care.

Ideal candidates have 2+ years of recent nursing experience, at least 1 year in home health, and CPR certification. Strong communication, organization, and knowledge of home care regulations are essential.

Qualifications

Graduate of an approved school of professional nursing.
CPR certification required and maintained.
Valid state driver license and automobile liability insurance.
Minimum two years nursing experience within last 36 months.
Minimum one year in home health.
Knowledge of federal/state/local regulations governing home care.
CPR certification required; license active.
Proficiency with Microsoft Office (Word/Excel).
In-office attendance required for supervision and teamwork.

Responsibilities

Coordinate home health services per standards and procedures.
Review/approve patient information for start of care, recertification, resumption of care, or evaluations.
Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
Schedule patients to field clinicians and maintain care coordination and communication.
