Interim HealthCare of the Twin Cities is seeking a Nurse Supervisor (RN) to oversee the nursing staff delivering home health and hospice services. You will collaborate with the Director of Nursing to implement processes that drive patient outcomes and maintain regulatory compliance.

Qualifications

• Must be a graduate of an accredited school of nursing and licensed as an RN in Minnesota with active license.
• Minimum 2 years RN experience, preferably in home care, plus at least 1 year supervisory experience.
• Ability to travel within the office-serviced geographic area with reliable transportation and valid driver's license.

Responsibilities

• Supervise, support, motivate, and develop nursing department staff.
• Recruitment support, interview and hire qualified staff.
• Onboard, orient, and provide ongoing education with directors.
• Manage staff performance, evaluations, and improvement plans.

The Home Care Nursing Supervisor, RN will work with our amazing Home Care staff toprovide oversight of the care and services furnished by employees to InterimHealthCare patients and clients. Collaborates with the Director of Nursing to develop and implement processes to ensure the efficient and effective achievement of patient/client outcomes

Pay and Benefits:

• Annual Salary Range: $104,312 - $124,384

A few must-haves for this position:

• Graduate of an accredited school of nursing and licensed to practice as an RN in the State of Minnesota. License is active and in good standing.
• Minimum two (2) years’ experience as an RN, preferably in home care, and at least one (1) year supervisory experience
• Able to travel within a geographic area serviced by the office. Reliable transportation, valid driver'slicense, and proof of auto insurance.
• CPR Certification

Some functions of what you would do:

• Supervises, supports, motivates, and develops professional and paraprofessional employees within the nursing department.
• Assists with recruitment, including the development of recruitment strategies, interviewing, and hiring of qualified staff.
• Collaborates with the Homecare Director of Nursing and the Director of Education to ensure the successful onboarding, orientation, and ongoing education of the employees within the nursing department.
• Participates in all aspects of performance management for assigned staff, including staff performance evaluations, issuing performance-based pay increases, coaching/counseling of staff, managing discipline, and the creation and management of employee performance improvement plans.
• Management and monitoring of time and attendance of assigned staff, including review and approval of time cards and bonuses, review and approval of sick days/PTO, and management of the schedule to ensure the department is adequately staffed at all times.
• Plans, assigns, and monitors the work of employees to ensure maximum productivity and the delivery of exceptional patient care.
• Reviews and monitors staff’s documentation for appropriateness, accuracy, timeliness, and quality.
• Ensures that appropriate action is taken to resolve identified employee concerns or complaints.
• Uses clinical decision making to provide oversight and guidance to the nursing team to efficiently and effectively manage the assigned patient population. Acts as a resource for all members of the interdisciplinary clinical care team.
• Assists in identifying Medicare appropriate needs for both skilled home care and hospice.
• Leads and/or actively participates in interdisciplinary, department, leadership or other meetings as needed and requested.
• Partners and effectively communicates with other supervisors, managers and employees as needed to support the entire Interim Healthcare business.
• Manages employees to achieve an acceptable level of customer service satisfaction.
• Engages with referral sources, patients, families, etc. as needed to facilitate exceptional customer service and resolve service failures, including investigation and documentation of complaints, in a timely manner.
• Participates in the Homecare Quality Assurance and Performance Improvement (QAPI) program and other quality improvement initiatives/projects related to process improvement, patient outcomes and the patient/family experience (star ratings, HHCAHPS, etc.)
• Addresses and resolves quality assurance issues in a timely manner with appropriate input from the Director of Nursing.
• Stays knowledgeable and up to date with Medicare Conditions of Participation and MN state homecare laws to ensure regulatory compliance at all times.
• Promotes fiscal responsibility and achievement of financial goals.
