Job summary

Enhabit Home Health & Hospice is seeking an experienced registered nurse to coordinate comprehensive home health services, ensure quality care, and liaise with physicians, patients, and caregivers.

The role requires a graduate nursing license, 2 years of recent nursing experience, 1 year in home health, CPR certification, and a valid driver's license. In-office attendance is essential to lead a care team and uphold standards.

Qualifications

• Graduate of an approved school of professional nursing with active license.
• Two years nursing experience within the last 36 months.
• One year experience in home health.
• CPR certification and valid state driver license.
• Knowledge of federal, state, and local home care regulations.

Responsibilities

• Coordinate home health services to meet standards of practice and company procedures.
• Review patient information at start of care, recertification, resumption, or evaluation visits.
• Facilitate relationships among physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and communicate with all stakeholders to maintain continuity of care.
