Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts. The role focuses on monitoring, follow-up, and clinical care management for dually-eligible enrollees with complex needs, coordinating health services and community resources to improve health outcomes and quality of life.

The position emphasizes integrating care across providers, managing risk, and leading an interdisciplinary team to develop and implement comprehensive care plans tailored to each enrollee.

Qualifications

• AS in nursing from an accredited program.
• RN able to independently serve people with complex medical, behavioral, and social needs.
• Prior experience in care coordination or case management preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage enrollees in homes and community settings to build an effective care management relationship.
• Coordinate with providers and community resources to ensure seamless communication and care transitions.
• Perform timely assessments (Comprehensive, MDS-HC or successor, crisis and risk).
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Oversee enrollee utilization of long-term services and supports; ensure they stay in their chosen location.
• Educate members about benefits and available services; promote self-management and preventive care.
