CareSource seeks an Integrated Care Clinical Manager (Massachusetts) to provide in-home complex care management for dually-eligible enrollees with medical, behavioral, and social needs. You will lead the interdisciplinary team, coordinate with providers, and ensure smooth transitions and effective utilization of long-term services.

The role requires an RN license in MA, an AS nursing degree, and experience in care coordination or case management.

Qualificações

• AS degree in nursing from an accredited program is required.
• RN licensure required to independently serve individuals with complex needs.
• Care coordination/case management experience for dual-eligible populations preferred.

Responsabilidades

• Engage enrollees in home and community settings for complex care management.
• Lead ICT to develop holistic care plans addressing medical and non-medical needs.
• Coordinate with providers and community resources to ensure seamless care.
