Qualifications

• Graduate of an accredited School of Nursing (ADN or BSN).
• Active RN license in good standing.
• Current BLS/CPR certification.
• Experience in long-term care, post-acute care, rehabilitation, or geriatric nursing preferred.
• English language proficiency for clear documentation and patient safety.
• Must meet health screenings including TST as required by state guidelines.

Responsibilities

• Deliver high-quality nursing care under supervision to achieve positive clinical outcomes.
• Perform comprehensive patient assessments and develop care plans.
• Delegate appropriate tasks to LPNs and CNAs.
• Maintain accurate, real-time documentation using PCC EHR.
• Collaborate with physicians and interdisciplinary teams for safe discharge planning.
