The role emphasizes leadership, mentorship, regulatory compliance, and precise documentation using PCC EHR to ensure safe, effective care and patient-family satisfaction.

Qualifications

• Education: ADN or BSN from an accredited school.
• Licensure: Active RN license in the state.
• Certifications: BLS/CPR current.
• Clinical Experience: Long-term care, post-acute, rehabilitative, or geriatric nursing preferred.
• Language: Proficient in English for documentation and safety.
• Health: Must pass required health screenings per state guidelines.

Responsibilities

• Perform comprehensive patient assessments and develop care plans.
• Administer medications and treatments and monitor patient progress.
• Lead and supervise LPNs and CNAs in accordance with state practice acts.
• Collaborate with physicians and interdisciplinary teams for care plans.
• Mentor and onboard new nursing staff to support team growth.
• Maintain accurate real-time documentation using PCC EHR and ensure safety.
