HealthFlex Home Health Services is seeking a Field Clinical Supervisor (RN) to blend hands-on patient care with field and office leadership across California. You’ll supervise a team of RN, LVN, and HHAs, train staff, conduct SOCs, and lead quality initiatives in a role that keeps you connected to bedside practice.

Qualifications

• Current, unrestricted California RN license.
• CPR certification required.
• 1–2 years of home health experience preferred with supervisory/mentoring experience.

Responsibilities

• Perform Start of Care (SOC) nursing evaluations and OASIS in compliance with Medicare.
• Conduct field supervisory visits for RNs, LVNs, and HHAs per policy and regs.
• Provide hands-on training and onboarding for new staff with shadow visits.
• Deliver direct patient care visits when needed to support caseloads.
• Document field encounters accurately in WellSky and manage scheduling coordination.
• Lead interdisciplinary case conferences and conduct utilization review.
• Oversee 5–7 full-time equivalents.
