Qualifikationen

• Licensed California Physical Therapist required, CPR certified.
• Experience supervising PTs/PTAs, and mentoring staff preferred.
• Knowledge of Medicare Conditions of Participation, OASIS, and CA home health rules.
• Proficiency with field-based start of care evaluations and documentation systems.

Aufgaben

• Perform Start of Care evaluations and SOC assessments.
• Conduct field supervisory visits for PTAs, PTs, and other staff.
• Lead and participate in annual performance evaluations and field training.
• Provide hands-on training, shadow visits, and competency validation for new staff.
• Deliver direct patient care when needed to cover caseloads.
• Document encounters accurately in the clinical system and ensure quality/compliance.
• Oversee 5-7 full-time equivalents and coordinate scheduling.
