Job summary

HealthFlex Home Health Services in California seeks a Field Clinical Supervisor to blend hands-on nursing with office-based case management, quality oversight, and staff development. This role supports Start of Care evaluations, field supervision, and ongoing education.

You will lead supervision and training for RNs/LVNs, conduct utilization reviews, participate in case conferences, and work to reduce rehospitalizations while staying connected to patient care.

Qualifications

• Current, unrestricted California RN license.
• Current CPR certification.
• 1–2 years of home health experience preferred; supervisory/mentoring experience desired.
• Working knowledge of Medicare COP, OASIS, and CA home health regulatory requirements.
• Proficiency with WellSky and Paylocity or willingness to train.

Responsibilities

• Complete SOC evaluations and start of care nursing assessments.
• Conduct field supervisory visits for clinicians per state requirements.
• Provide hands-on training and shadow visits for new staff.
• Deliver direct patient care when needed to support caseloads.
• Document encounters in the record accurately and timely.
• Oversee 5–7 full-time equivalents.
• Manage care plans, coordinate with interdisciplinary team, and oversee scheduling.
