Job summary

HealthFlex Home Health & Hospice is seeking an experienced RN to step into the Field Clinical Supervisor role. You will supervise a team of RNs, LVNs, and HHAs, train staff in the field, and remain actively engaged in Start of Care evaluations to keep patient care grounded in current practice.

The role blends direct field nursing with office duties, including utilization review and scheduling coordination, and is a salaried, hybrid position requiring travel to patient homes and office presence.

Qualifications

• Current, unrestricted California RN license.
• CPR certification.
• Minimum 1–2 years of home health experience; supervisory or mentoring preferred.
• Working knowledge of COP, OASIS, and California home health regulations.

Responsibilities

• Field supervisory visits for clinicians per California requirements.
• Lead start of care evaluations and case conferences.
• Provide hands-on training and shadow visits for new staff.
• Coordinate with office for scheduling, utilization review, and QA.
