Job summary

HealthFlex Home Health Services seeks an experienced RN for Field Clinical Supervisor to lead field and office-based quality, while staying connected to bedside care through SOC evaluations, shadow visits, and staff training.

In a hybrid, travel-heavy role, you will supervise 5-7 clinicians, oversee utilization and rehospitalization prevention, and contribute to case conferences and education across the region.

Qualifications

• Current, unrestricted California RN license in good standing.
• Current CPR certification required.
• Experience in home health and supervisory/mentoring roles preferred.

Responsibilities

• Complete SOC skilled nursing evaluations and OASIS per Medicare rules.
• Conduct field supervisory visits for clinicians per California requirements.
• Lead annual performance evaluations with field observations and feedback.
• Provide hands-on training, shadow visits, and competency validation.
• Perform utilization review to ensure care aligns with the plan of care.
