Job summary

HealthFlex Home Health & Hospice is seeking a Field Clinical Supervisor in California. This role blends hands-on Start of Care nursing with field supervision, staff mentoring, and quality oversight to improve patient outcomes while staying connected to bedside care.

You will supervise a team of clinicians, lead case conferences, and ensure adherence to regulatory requirements, with a blended office and field schedule.

Qualifications

• Current, unrestricted California RN license in good standing.
• Current CPR certification.
• Minimum 1–2 years of home health experience; supervisory/mentoring experience preferred.
• Working knowledge of Medicare Conditions of Participation, OASIS, and California home health regulatory requirements.
• Proficiency with WellSky (or willingness to train) and Paylocity or similar HRIS.

Responsibilities

• Complete Start of Care (SOC) nursing evaluations and OASIS in accordance with Medicare rules.
• Conduct field supervisory visits for RNs, LVNs, and aides per regulatory requirements.
• Provide hands-on training and onboarding for newly hired RNs and LVNs.
• Deliver direct patient care visits as needed to support caseloads.
• Document field encounters accurately in WellSky.
• Oversee 5–7 full-time equivalents.
• Manage caseload and coordinate with interdisciplinary team.
• Perform utilization review to ensure plan of care aligns with authorizations and medical necessity.
• Lead case conferences and contribute to quality initiatives.
