Job summary

Qualifications

• Current, unrestricted California RN license.
• Current CPR certification.
• Minimum 1–2 years of home health experience preferred; supervisory or training experience preferred.
• Working knowledge of Medicare Conditions of Participation, OASIS, and California home health requirements.
• Proficiency with WellSky and Paylocity or willingness to train.
• Valid California driver's license and reliable transportation.
• Strong written and verbal communication; ability to mentor staff.
• Ability to manage field and office priorities.

Responsibilities

• Complete SOC nursing evaluations including the OASIS per policy and regulations.
• Conduct field supervisory visits for RNs, LVNs, HHAs, and staff for performance evaluations.
• Provide hands-on training and onboarding for new RNs/LVNs with shadow visits.
• Deliver direct patient care visits as needed to support caseload.
• Document encounters in WellSky accurately and timely.
• Oversee 5–7 FTEs.
• Manage care plans and coordinate with the interdisciplinary team, schedulers, and concierges.
• Perform utilization review to align visits with plan of care and authorization.
• Lead case conferences and review patient progress and needs.
• Conduct root cause analyses for rehospitalizations and educate staff to prevent them.
• Travel within service territory; hybrid role with time in office.
