Job summary

HealthFlex Home Health Services is seeking a Field Clinical Supervisor, a salaried RN role combining direct patient care with office-based case management and quality functions. You will supervise a team of clinicians while staying in the field for SOCs and patient visits.

The position emphasizes leadership grounded in current practice, with responsibilities across utilization review, staff education, scheduling, and participation in case conferences to improve patient outcomes.

Qualifications

• Current, unrestricted California RN license.
• Current CPR certification.
• Minimum 1–2 years of home health experience preferred.
• Supervisory/training experience preferred.
• Knowledge of Medicare Conditions of Participation and OASIS is preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform Start of Care evaluations and OASIS per policy.
• Conduct field supervisory visits for clinical staff per CA requirements.
• Provide hands-on training and mentorship to RNs, LVNs, HHAs.
• Lead field-based performance evaluations and quality monitoring.
• Coordinate scheduling and case load management with office staff.
• Perform utilization review and support rehospitalization prevention.
