Job summary

HealthFlex Home Health Services is seeking an experienced RN for the Field Clinical Supervisor role, blending direct bedside work with field supervision and office-based quality functions across our communities. Qualifications

• Current, unrestricted California Registered Nurse (RN) license
• Current CPR certification
• Minimum 1–2 years of home health experience preferred; supervisory/training experience preferred
• Working knowledge of Medicare Conditions of Participation, OASIS, and California home health regulatory requirements
• Proficiency with WellSky or willingness to train
• Valid California driver's license and reliable transportation

Responsibilities

• Complete Start of Care (SOC) nursing evaluations including the comprehensive assessment and OASIS
• Conduct field supervisory visits for LVNs, home health aides, and other staff
• Conduct field supervisory visits for RNs, LVNs, and aides for annual performance reviews
• Provide hands-on training and onboarding for new RNs and LVNs
• Deliver direct patient care visits as needed to support caseload
• Document field encounters accurately in WellSky
• Oversee 5–7 Full Time Equivalents
• Manage care plans and coordinate with the interdisciplinary team
• Monitor scheduling activity and enforce scheduling policies
• Lead interdisciplinary case conferences and perform utilization review
• Review patient complaints and conduct root-cause analysis for rehospitalizations
• Travel to patient homes and maintain infection control standards
• Hybrid role requiring travel and office time for training and quality functions
• Respond to Livanta appeals as needed
