Enlyte in Sacramento, CA is seeking a Field Case Manager to coordinate care for injured workers. You will work with treating physicians, employers, and patients to create and implement effective treatment plans and safely return individuals to work.

The role requires an active RN license, case management certifications, and travel within the assigned geography. Benefits include a comprehensive program and generous paid time off.

Qualifications

• RN license must be active and in good standing.
• URAC-recognized case management certification (CCM, CDMS, CRC, CRRN or COHN/COHN-S, RN-BC, ACM, CMAC, CMC).
• 2+ years of clinical practice; workers' compensation experience preferred.

Responsabilités

• Develop and implement treatment plans with physicians and employers.
• Coordinate in-person appointments and travel within geography.
• Monitor patient progress and adjust goals to maximize recovery and return-to-work.
• Manage caseload independently and document status updates for clients.
