Job summary

Enlyte is seeking a Field Case Manager in the Jacksonville, FL area. This full-time, hybrid role requires travel to in-person patient appointments and working closely with treating physicians, employers, and injured workers to develop and execute effective treatment plans.

Qualifications

• Associates Degree or Bachelor's Degree in Nursing or related field.
• 2+ years clinical practice preferred.
• URAC-recognized certification in case management (CCM, CDMS, CRC, CRRN or COHN) preferred.

Responsibilities

• Demonstrate knowledge, skills, and competency in the application of case management standards of practice.
• Advise on treatment plans with physicians/providers and employers.
• Interview disabled persons to assess recovery and occupational considerations.
• Collaborate with physicians/providers to create tailored treatment plans.
• Modify job duties with employers to facilitate early return to work.
• Manage workload independently and prioritize cases.
• Attend injured worker appointments and prepare status updates for customers.
