Enlyte is seeking a Field Case Manager to coordinate care for injured workers and auto accident patients in the Nashville area. The role requires RN licensure, independent case management, and collaboration with treating physicians and employers to return patients to work.

Qualifications

• Associates or Bachelor’s Degree in Nursing or related field.
• 2+ years clinical practice preferred; workers’ compensation experience preferred.
• Active RN license in good standing; URAC-recognized case management certification options (CCM, CDMS, CRC, CRRN or COHN, COHN-S, RN-BC, ACM, CMAC, CMC).
• Travel required; reliable transportation to travel to in-person appointments.
• Must have reliable internet and transportation to assigned geography.

Responsibilities

• Manage case load independently and prioritize cases effectively.
• Collaborate with physicians/providers, employers, and patients to develop treatment plans.
• Interviews and assesses recovery, determines occupational vs non-occupational factors.
• Modify job duties with employers to facilitate early return to work.
• Prepare status updates and communicate progress to customers.
