RN, Field Care Manager (Maternal-Child Health) - Northern/Winchester, Virginia

Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) to assess and evaluate high-acuity members' needs, guiding them towards and facilitating interaction with appropriate resources for their care and well-being, particularly for pregnant and postpartum members. This role involves varied work assignments requiring interpretation and independent decision-making.
- Conduct health risk assessments across the care continuum.
- Create individualized care plans and participate in interdisciplinary team meetings.
- Employ strategies to manage physical, environmental, and psycho-social health issues.
- Identify and resolve barriers to effective care.
- Monitor members' progress towards desired outcomes through assessments and evaluations.
- Understand departmental and organizational strategies and objectives.
- Make autonomous decisions regarding work methods, often in ambiguous situations, with minimal guidance.
- Collaborate with internal and external providers to meet member needs.
- Ensure compliance with contractual and regulatory standards.
- Follow established guidelines and procedures.

Required Qualifications
- Reside in the Northern/Winchester Region of Virginia, including counties and independent cities such as Winchester, Frederick, Clarke, Warren, Shenandoah, Page, Rappahannock, Culpeper, Fauquier, Prince William, Fairfax, Alexandria, Arlington, Falls Church, Loudoun, Manassas, and Manassas Park.
- Active RN license in Virginia without disciplinary actions.
- At least 2 years of healthcare or case management experience.
- Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
- Proficient in Microsoft Word, Outlook, and Excel.
- Knowledge of community health and social services.
- Excellent communication and interpersonal skills.
- Ability to work independently within scope of practice.
- Experience with electronic medical records and web navigation.
- Must pass TB screening and have a valid driver’s license with insurance for travel to member homes and facilities.
Preferred Qualifications
- Bachelor's degree in Nursing (BSN).
- 3-5 years of in-home assessment and care coordination.
- Lactation certification, preferably as a lactation consultant.
- Experience in health promotion, coaching, wellness, and managed care.
- Case Management Certification (CCM).
- Motivational Interviewing certification or knowledge.
- Bilingual abilities, especially in Spanish, Arabic, Vietnamese, Amharic, Urdu, or others.
Additional Information
- This is a remote role with regional travel (50-75%) to meet members and providers, with occasional onsite meetings in Glen Allen, VA.
- Work hours: Monday–Friday, 8:00 am–5:00 pm EST.
- Must meet internet speed requirements and work in a dedicated, interruption-free space.

Compensation and Benefits
Annual salary: $71,100 - $97,800, plus potential bonuses based on performance. Benefits include medical, dental, vision, 401(k), paid time off, disability, life insurance, and more.
