RN, Field Care Manager(Maternal-Child Health) - Northern/Winchester, Virginia

Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high acuity member's needs and requirements to achieve and/or maintain an optimal wellness state by guiding members and families toward appropriate resources for the care and well-being of members who are pregnant and postpartum. The work assignments are varied and often require interpretation and independent decision-making.
Responsibilities include:
- Conducting health risk assessments across the continuum of care for members.
- Assessing members, creating individualized care plans, and participating in interdisciplinary care team meetings.
- Employing strategies to manage physical, environmental, and psycho-social health issues.
- Identifying and resolving barriers to effective care.
- Monitoring members’ progress towards desired outcomes through ongoing assessments and evaluations.
- Understanding departmental, segment, and organizational strategies and objectives, including their linkages to related areas.
- Making work method decisions, often in ambiguous situations, with minimal supervision.
- Collaborating with internal and external providers or departments to meet member needs.
- Ensuring compliance with contractual and regulatory requirements.
- Following established guidelines and procedures.
Qualifications:
Required:
- Must reside in the Northern/Winchester Region in Virginia, including counties and independent cities such as Winchester, Frederick, Clarke, Warren, Shenandoah, Page, Rappahannock, Culpeper, Fauquier, Prince William, Fairfax, Alexandria, Arlington, Falls Church, Loudoun, Manassas, and Manassas Park.
- Active RN license in Virginia without disciplinary action.
- At least 2 years of healthcare and/or case management experience.
- Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
- Intermediate to advanced skills in Microsoft Office (Word, Outlook, Excel).
- Knowledge of community health, social service agencies, and community resources.
- Excellent communication and interpersonal skills.
- Ability to work independently within scope and use electronic medical records.
- Ability to travel regionally for face-to-face assessments.
- Must have a valid driver’s license and personal vehicle insurance, with coverage meeting state minimums.
- This role involves TB screening and compliance with Humana’s driver safety program.
Preferred:
- Bachelor’s of Science in Nursing (BSN).
- 3-5 years of in-home assessment and care coordination experience.
- Experience with lactation consulting and health promotion.
- Previous managed care experience.
- Certification in Case Management (CCM).
- Motivational Interviewing certification or knowledge.
- Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or other languages.
Additional Information:
- This is a remote position requiring travel (50-75%).
- Work hours: Monday–Friday, 8:00 am–5:00 pm EST.
- Applicants must pass a language proficiency assessment if speaking a language other than English.

- Compensation: $71,100 - $97,800 annually, with potential bonuses based on performance.
