RN, Field Care Manager(Maternal-Child Health) - Northern/Winchester, Virginia

Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high acuity member's needs and requirements to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources, especially for members who are pregnant and postpartum. The RN, Field Care Manager Nurse 2 (Maternal-Health) will have varied work assignments requiring interpretation and independent decision-making.

  • Responsible for health risk assessments across the continuum of care for members.
  • Creates individualized care plans and attends interdisciplinary care team meetings.
  • Employs various strategies and techniques to manage members' physical, environmental, and psycho-social health issues.
  • Identifies and resolves barriers to effective care.
  • Monitors member progress towards desired outcomes through assessments and evaluations.
  • Understands departmental, segment, and organizational strategies and objectives, including their linkages.
  • Makes decisions regarding work methods, often in ambiguous situations, with minimal supervision.
  • Collaborates with internal and external providers to meet member needs.
  • Ensures compliance with contractual and regulatory requirements.
  • Follows established guidelines and procedures.

Required Qualifications

  • Must reside in the Northern/Winchester Region in Virginia, including counties and independent cities such as Winchester, Frederick, Clarke, Warren, Shenandoah, Page, Rappahannock, Culpeper, Fauquier, Prince William, Fairfax, Alexandria, Arlington, Falls Church, Loudoun, Manassas, and Manassas Park.
  • Active RN license in Virginia without disciplinary action.
  • At least 2 years of healthcare and/or case management experience.
  • Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
  • Intermediate to advanced skills in Microsoft Office applications.
  • Knowledge of community health and social service agencies.
  • Strong communication and interpersonal skills.
  • Ability to work independently within scope.
  • Proficiency with electronic medical records and web navigation.
  • This role involves patient-facing activities and TB screening.
  • Must have a valid driver’s license and personal vehicle insurance, with ability to travel regionally.

Preferred Qualifications

  • Bachelor’s degree in Nursing (BSN).
  • 3-5 years of in-home assessment and care coordination.
  • Lactation certification or experience.
  • Experience in health promotion, coaching, wellness, and managed care.
  • Certification in Case Management (CCM).
  • Motivational Interviewing certification or knowledge.
  • Bilingual abilities, especially in Spanish, Arabic, Vietnamese, Amharic, or Urdu.
Additional Information

Position is remote with travel requirements of 50-75%. Work hours are Monday to Friday, 8:00 am – 5:00 pm EST. Must meet internet speed requirements and work in a dedicated, interruption-free space. Occasional onsite meetings may occur in Glen Allen, VA. The role involves use of HireVue for interviews and may require travel to Humana offices for training or meetings.
