Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) to assess and evaluate high-acuity members' needs to promote and maintain optimal wellness. This role involves guiding members and families to appropriate resources for the care of pregnant and postpartum members. Work assignments are varied and often require independent judgment.

• Conduct health risk assessments across the care continuum.
• Create individualized care plans and participate in interdisciplinary team meetings.
• Use various strategies to manage physical, environmental, and psycho-social health issues.
• Identify and resolve barriers to effective care.
• Monitor member progress and adjust care plans accordingly.
• Understand organizational strategies and their linkages.
• Make independent decisions regarding work methods, sometimes in ambiguous situations.
• Collaborate with internal and external providers to meet member needs.
• Ensure compliance with contractual and regulatory standards.
• Follow established guidelines and procedures.

Required Qualifications

• Must reside in the Northern/Winchester Region of Virginia, including Winchester, Frederick, Clarke, Warren, Shenandoah, Page, Rappahannock, Culpeper, Fauquier, Prince William, Fairfax, Fairfax City, Alexandria, Arlington, Falls Church, Loudoun, Manassas, and Manassas Park.
• Active RN license in Virginia without disciplinary actions.
• At least 2 years of healthcare and/or case management experience.
• Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
• Proficient with Microsoft Office applications.
• Knowledge of community health and social services.
• Excellent communication and interpersonal skills.
• Ability to work independently within scope.
• Experience with electronic medical records.
• Must pass TB screening and have a valid driver’s license with insurance for travel.

Preferred Qualifications

• Bachelor's Degree in Nursing (BSN).
• 3-5 years in-home assessment and care coordination.
• Lactation certification, preferably as a lactation consultant.
• Experience in health promotion and wellness coaching.
• Managed care experience.
• Case Management Certification (CCM).
• Motivational Interviewing certification.
• Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or others.

Additional Information

• This is a remote role requiring regional travel (50-75%).
• Work hours are Monday to Friday, 8:00 am – 5:00 pm EST.
• Applicants must complete a language proficiency assessment if speaking with members in non-English languages.
• Internet must meet minimum speed requirements for remote work.
• Use HireVue for interviews.
• Position may require occasional onsite meetings in Glen Allen, VA.

Salary and Benefits

Annual salary ranges from $71,100 to $97,800, with eligibility for bonuses. Benefits include health insurance, 401(k), paid time off, disability, life insurance, and wellness programs.
