RN, Field Care Manager (Maternal-Child Health) - Northern/Winchester, Virginia

Humana Healthy Horizons in Virginia is seeking an RN, Field Care Manager Nurse 2 (Maternal-Health) to assess and evaluate high-acuity members' needs, guiding them toward and facilitating interaction with appropriate resources for the care of pregnant and postpartum members. The role involves varied work assignments requiring interpretation and independent decision-making.
Responsibilities include:
- Conducting health risk assessments across the care continuum.
- Creating individualized care plans and participating in interdisciplinary care team meetings.
- Employing strategies to manage physical, environmental, and psycho-social health issues.
- Identifying and resolving barriers to effective care.
- Monitoring member progress and adjusting care as needed.
- Understanding organizational strategies and objectives.
- Making autonomous decisions within scope, with minimal guidance.
- Collaborating with internal and external providers.
- Ensuring compliance with contractual and regulatory standards.
- Following established guidelines and procedures.
Qualifications:
Required:
- Reside in the Northern/Winchester Region of Virginia, including specified counties and cities.
- Active RN license in Virginia without disciplinary action.
- At least 2 years of healthcare and/or case management experience.
- Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
- Proficient with Microsoft Office and electronic medical records.
- Knowledge of community health and social services.
- Excellent communication and interpersonal skills.
- Ability to work independently and travel regionally.
- Valid driver’s license and vehicle insurance.
Preferred:
- Bachelor’s Degree in Nursing (BSN).
- 3-5 years of in-home assessment and care coordination experience.
- Experience as a lactation consultant, health promotion, coaching, or wellness.
- Managed care experience and CCM certification.
- Motivational Interviewing certification.
- Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or other languages.
Additional Information:
- This is a remote position requiring travel (50-75%).
- Work hours: Monday-Friday, 8:00 am - 5:00 pm EST.
- Must meet internet requirements and work from a dedicated, interruption-free space.
- Participation in Humana’s TB screening and driver safety programs required.
- Interview via HireVue technology.
- Salary range: $71,100 - $97,800 annually, with potential bonuses.
