Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) who will assess and evaluate high acuity members' needs to promote optimal wellness. The role involves guiding members and families, facilitating interactions with appropriate resources for pregnant and postpartum members, and interpreting and determining the best courses of action independently.
- Conduct health risk assessments across the care continuum.
- Create individualized care plans and participate in interdisciplinary team meetings.
- Utilize various strategies to manage physical, environmental, and psycho-social health issues.
- Identify and resolve barriers to effective care.
- Monitor member progress through ongoing assessments and evaluations.
- Align work with departmental, segment, and organizational strategies and objectives.
- Make autonomous decisions regarding work methods in ambiguous situations with minimal guidance.
- Collaborate with internal and external providers to meet member needs.
- Ensure compliance with contractual and regulatory standards.
- Follow established guidelines and procedures.

Required Qualifications
- Reside in the Northern/Winchester Region of Virginia, including Winchester, Frederick, Clarke, Warren, Shenandoah, Page, Rappahannock, Culpeper, Fauquier, Prince William, Fairfax, Fairfax City, Alexandria, Arlington, Falls Church, Loudoun, Manassas, and Manassas Park.
- Active RN license in Virginia without disciplinary actions.
- At least 2 years of healthcare and/or case management experience.
- Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
- Proficient with Microsoft Office Suite and electronic medical records.
- Knowledge of community health and social services.
- Excellent communication and interpersonal skills.
- Ability to work independently within role scope.
- Eligible for TB screening and travel to region-based facilities and homes.
- Possess a valid driver’s license and personal vehicle insurance.
Preferred Qualifications
- Bachelor’s degree in Nursing (BSN).
- 3-5 years in-home assessment and care coordination.
- Lactation certification (e.g., lactation consultant).
- Experience with health promotion and wellness coaching.
- Managed care experience.
- Certification in Case Management (CCM).
- Motivational Interviewing certification or knowledge.
- Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or other languages.
Additional Information
- This is a remote position requiring travel (50-75%).
- Work hours are Monday-Friday, 8:00 am - 5:00 pm EST.
- Must pass a language proficiency assessment if speaking with members in languages other than English.
- Internet speed requirements: minimum 25 Mbps download and 10 Mbps upload.
- Occasional onsite meetings in Glen Allen, VA, may be required.
- Use of HireVue for interviews.
