Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health). The role involves assessing high-acuity members' needs, guiding them and their families to appropriate resources, and facilitating interactions to support their well-being, especially for pregnant and postpartum members. The work requires interpretation and independent judgment to determine appropriate actions.
- Conduct health risk assessments across the care continuum.
- Create individualized care plans and participate in interdisciplinary team meetings.
- Employ diverse strategies to manage members' physical, environmental, and psycho-social health issues.
- Identify and resolve barriers to effective care.
- Monitor members' progress toward health outcomes through ongoing assessments.
- Understand organizational strategies and how they relate to your role.
- Make autonomous decisions regarding work methods, occasionally in ambiguous situations, with minimal supervision.
- Collaborate with internal and external providers to meet member needs.
- Ensure compliance with contractual and regulatory standards.
- Follow established guidelines and procedures.

Required Qualifications
- Must reside in the Northern/Winchester Region of Virginia, including Winchester, Frederick, Clarke, Warren, Shenandoah, Page, Rappahannock, Culpeper, Fauquier, Prince William, Fairfax, Fairfax City, Alexandria, Arlington, Falls Church, Loudoun, Manassas, and Manassas Park.
- Active RN license in Virginia without disciplinary actions.
- At least 2 years of healthcare and/or case management experience.
- Minimum 1 year of acute care experience in obstetrics, women’s health, labor and delivery, NICU, or clinical triage.
- Intermediate to advanced skills in Microsoft Word, Outlook, and Excel.
- Knowledge of community health and social service resources.
- Excellent communication and interpersonal skills, with the ability to build rapport quickly.
- Ability to work independently within your scope.
- Experience with electronic medical records and other software applications.
- Ability to travel regionally for face-to-face assessments.
- Must meet TB screening requirements and have a valid driver’s license with appropriate insurance.
Preferred Qualifications
- Bachelor's of Science in Nursing (BSN).
- 3-5 years of in-home assessment and care coordination experience.
- Lactation certification or experience.
- Experience in health promotion, coaching, and wellness.
- Managed care experience.
- Certification in Case Management (CCM).
- Knowledge of Motivational Interviewing.
- Bilingual abilities in English and another language such as Spanish, Arabic, Vietnamese, Amharic, or Urdu.
Additional Information
- This is a remote position requiring travel.
- Travel is approximately 50-75% to meet members and providers, with occasional onsite meetings in Glen Allen, VA.
- Work hours are Monday–Friday, 8:00 am – 5:00 pm EST.
- Language proficiency assessments are required for non-English speakers.

Pay Range
$71,100 - $97,800 annually, based on location and experience. Eligible for performance bonuses.
