RN, Field Care Manager (Maternal-Child Health) - Northern/Winchester, Virginia

Humana Healthy Horizons in Virginia is seeking a RN, Field Care Manager Nurse 2 (Maternal-Health) to assess and evaluate high acuity members' needs, guiding them towards and facilitating interaction with appropriate resources for their care and well-being, especially for pregnant and postpartum members. The work involves varied assignments requiring interpretation and independent decision-making.

  • Conduct health risk assessments across the care continuum.
  • Create individualized care plans and participate in interdisciplinary team meetings.
  • Employ diverse strategies to manage physical, environmental, and psycho-social health issues.
  • Identify and resolve barriers to effective care.
  • Monitor member progress towards health outcomes through ongoing assessments.
  • Understand organizational strategies and linkages to related areas.
  • Make autonomous work decisions, sometimes in ambiguous situations, with minimal guidance.
  • Collaborate with internal and external providers to meet member needs.
  • Ensure compliance with contractual and regulatory standards.
  • Follow established guidelines and procedures.

Required Qualifications

  • Reside in the Northern/Winchester Region of Virginia, including Winchester, Frederick, Clarke, Warren, Shenandoah, Page, Rappahannock, Culpeper, Fauquier, Prince William, Fairfax, Fairfax City, Alexandria, Arlington, Falls Church, Loudoun, Manassas, and Manassas Park.
  • Active RN license in Virginia without disciplinary action.
  • At least 2 years of healthcare and/or case management experience.
  • Minimum 1 year of acute care experience in obstetrics, women’s care, labor and delivery, NICU, or clinical triage.
  • Proficient with Microsoft Word, Outlook, and Excel.
  • Knowledge of community health and social services.
  • Excellent communication and interpersonal skills.
  • Ability to work independently within scope.
  • Experience with electronic medical records and web navigation.
  • Must be able to travel regionally for face-to-face assessments.
  • Must pass TB screening and hold a valid driver’s license with insurance coverage.

Preferred Qualifications

  • Bachelor's in Nursing (BSN).
  • 3-5 years of in-home assessment and care coordination.
  • Lactation certification, preferably as a lactation consultant.
  • Experience in health promotion, coaching, and wellness.
  • Managed care experience.
  • Certification in Case Management (CCM).
  • Motivational Interviewing certification or knowledge.
  • Bilingual in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or others.

Additional Information

  • This is a remote role requiring travel.
  • Travel expected 50-75% to meet members and providers, with occasional onsite meetings in Glen Allen, VA.
  • Work hours: Monday – Friday, 8:00 am – 5:00 pm EST.
  • Language proficiency assessment required for non-English speakers.

  • Minimum internet speed: 25 Mbps download, 10 Mbps upload.
  • Wireless, wired, or DSL connections recommended; satellite or cellular only with approval.
  • Work from a dedicated, interruption-free space to protect PHI/HIPAA data.

Interview Process

Hiring involves a HireVue interview for assessing relevant skills and experience.

Scheduled weekly hours: 40

Estimated annual salary: $71,100 - $97,800, with potential bonuses based on performance.
