Job summary

UnitedHealth Group's UnitedHealthcare segment seeks an RN Clinical Care Coordinator to manage a panel of members with complex medical and behavioral needs, coordinating medical, behavioral, and social support to improve care quality and appropriate utilization.

Role is field-based with a home-based office; you will travel up to 75% and mileage is reimbursed. Candidates must hold MA RN licensure and demonstrate strong care coordination and EMR literacy.

Qualifications

• High School Diploma or GED required.
• Current and unrestricted MA RN license.
• 2+ years of clinical experience.
• Proficient with MS Office (Word, Excel, Outlook).
• Access to a quiet home workspace with PHI security.
• Reliable internet connection at home or in territory traveled.

Responsibilities

• Conduct needs assessments face-to-face and via telecommunication.
• Develop and implement person-centered care plans addressing medical, behavioral, and social needs.
• Collaborate with internal care teams, providers, and community resources to execute care plans.
• Educate and coach members for self-management of care needs and lifestyle changes.
• Support proactive discharge planning and care transitions following ER visits or SNF admissions.
• Advocate for members to ensure needs and choices are represented by the care team.
