Based in Carroll County with the flexibility to work remotely as a field-based role; strong teamwork with Case Management, Utilization Review, and Behavioral Health is essential for success.

Qualifications

• Current Nursing License (RN).
• 2+ years of clinical RN experience.
• 1+ years of MS Office experience (Word/Excel/Outlook).
• Excellent organization, communication and time management skills.
• Reside in or within commutable driving distance to Carroll County.
• Access to reliable transportation and valid US driver’s license with travel up to 75% within territory.

Responsibilities

• Apply clinical knowledge to coordinate care for Medicare/Medicaid members.
• Incorporate evidence-based clinical practices into care coordination.
• Deliver member-centered, self-management support and education.
• Ensure appropriate utilization of health benefits.
• Conduct telephonic and face-to-face assessments per policy and timelines.
• Advise and advocate for members to resolve access barriers to care.
• Coordinate referrals to community resources and providers.
• Develop and maintain individual care plans when indicated.
