Job summary

UnitedHealthcare is hiring a Field-based Care Coordinator (RN) to coordinate Medicare/Medicaid member care across acute, home, and community settings.

You will apply clinical knowledge, educate members, and advocate to resolve access barriers while collaborating with Case Management, Utilization Review, and Behavioral Health teams. The role requires travel up to 75% within the assigned territory.

Qualifications

• Current Nursing License (RN)
• 2+ years of clinical RN experience
• 1+ years of MS Office experience (Word, Excel, Outlook)
• Excellent organization, communication and time management skills
• Reside in or within commutable driving distance to Monroe County
• Access to reliable transportation and valid US driver's license with ability to travel up to 75% within assigned territory

Responsibilities

• Apply clinical knowledge to efficient case management for Medicare/Medicaid beneficiaries
• Incorporate evidence-based clinical practices into care coordination
• Deliver member-centered self-management support and education
• Ensure appropriate utilization and consistent application of health benefits
• Conduct telephonic and face-to-face assessments per policy and timelines
• Serve as member advocate to resolve barriers to care
• Coordinate referrals to community resources
• Develop and maintain member-centered care plans
• Collaborate with Care Coordination Team to improve care delivery
• Monitor hospital encounters and provide education/interventions as needed
• Refer non-qualifying decisions to manager for review
• Participate in Quality Improvement processes
• Use Health Plan software to document service requests and care activities
• Maintain time-sensitive documentation for regulatory compliance
• Coordinate services per Health plan policy
• Perform other duties as assigned
