Job summary

Elevance Health is seeking an LTSS Service Coordinator – RN Clinician (Case Manager) in Columbus, OH. This field-based role travels to client sites and requires an active RN license in Ohio.

You will assess and coordinate care for members with chronic illnesses, develop care plans, and collaborate with physicians and care teams to optimize health outcomes. The position emphasizes person-centered care, cross-functional teamwork, and adherence to state policies.

Qualifications

• Requires a high school diploma or GED and at least 3 years of experience with individuals with chronic illnesses.
• Current, active RN license in Ohio state(s) required.
• BA/BS in Health/Nursing preferred.
• Excellent communication and coordination skills.

Responsibilities

• Perform telephonic and face-to-face assessments to identify, evaluate, coordinate and manage member needs.
• Manage members with chronic illnesses to ensure cost-effective, efficient use of health benefits.
• Develop and monitor individual care plans with short and long term goals.
• Coordinate authorizations/referrals for services within benefits structure or through arrangements.
• Interface with Medical Directors and care teams on development of person-centered care plans.
