Job summary

Elevance Health in Seven Hills, OH, is seeking an LTSS Service Coordinator – RN Clinician (RN Case Manager) to manage member cases and develop care plans.

You’ll perform assessments, coordinate care for high-risk members with chronic illnesses, and collaborate with Medical Directors and care teams to ensure timely access to services. This field-based role offers a comprehensive benefits package and opportunities to impact members and communities.

Qualifications

• Active RN license in Ohio required.
• Minimum 3 years of case management experience.
• High school diploma or GED required; BA/BS preferred.

Responsibilities

• Perform telephonic and face-to-face functional assessments for members.
• Identify members for high-risk complications and coordinate care.
• Manage members with chronic illnesses, co-morbidities, and/or disabilities.
• Obtain thorough and accurate member history to develop individual care plans.
• Establish short and long-term goals in collaboration with members and their support systems.
• Develop care plans for services and ensure member access to those services.
• Facilitate authorizations/referrals for utilization of services as appropriate.
• Interface with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams on care management.
• Assist in problem-solving with providers, claims, or service issues.
