Job summary

UnitedHealthcare is seeking an RN Clinical Coordinator to manage a panel of members with complex medical and behavioral needs in Salem, NJ, and surrounding counties. The role involves substantial field work (about 80% of time) visiting members in homes or long-term care facilities and coordinating care with the internal team and community resources.

The ideal candidate will have NJ RN licensure, 2+ years of clinical experience, and ability to travel within the territory.

Qualifications

• Current, unrestricted RN license in New Jersey.
• 2+ years of clinical experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Driver’s license and ability to travel up to 80% within assigned territory.

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans.
• Partner with internal care team, providers, and community resources.
• Provide education and coaching to support self-management.
• Coordinate care transitions after ER visits or SNF admissions.
• Advocate for members and families as needed.

Registered Nurse License (NJ)

CCM certification

BSN preferred
