Job summary

UnitedHealth Group seeks a qualified RN Clinical Care Coordinator to manage a patient panel with complex medical and behavioral needs. The role emphasizes in-home and facility-based field visits, coordinating medical, behavioral, and social needs to optimize care and reduce unnecessary utilization.

Expect about 80% of time in the field visiting members and providers, with flexibility to travel across Salem County and surrounding areas in NJ.

Qualifications

• Current, unrestricted RN licensure in New Jersey.
• 2+ years of clinical experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Driver's license and reliable transportation; travel up to 80% within assigned territory.
• Preferred: BSN or higher.
• CCM certification preferred.
• Experience in community case management a plus.

Responsibilities

• Engage members face-to-face and/or telephonically to complete comprehensive needs assessments.
• Develop and implement person-centered care plans addressing medical, behavioral, functional and social needs.
• Collaborate with care team, providers and community resources to execute care plans.
• Provide education and coaching to support member self-management.
• Support proactive discharge planning and Care Transition after ER, inpatient, or SNF events.
• Advocate for members and families to ensure needs are represented and respected.
