Job summary

UnitedHealth Group’s UnitedHealthcare unit seeks an RN Clinical Care Coordinator to manage a panel of members with complex medical and behavioral needs. The role emphasizes field-based care coordination, home visits, and collaboration with care teams to improve quality and access.

Expect to spend approximately 80% of time in the field visiting members in their homes or long-term care facilities; requires NJ RN license, 2+ years clinical experience, and 80% travel within Mercer and surrounding

Qualifications

• Active NJ RN license
• 2+ years clinical experience
• 1+ years MS Office experience
• Ability to travel up to 80% within assigned territory

Responsibilities

• Engage members face-to-face and/or telephonically for comprehensive needs assessments
• Develop and implement person-centered care plans for chronic conditions and social determinants of health
• Collaborate with internal care team, providers, and community resources to execute care plans
• Educate and coach members to support self-management and healthy lifestyle changes
• Coordinate Care Transitions after ER/inpatient/SNF admissions
• Advocate for members to ensure needs and choices are represented by the care team
