Qualifications

• Current, unrestricted RN license in New Jersey.
• 2+ years of clinical experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Driver's license and reliable transportation; travel up to 80% of time.

Responsibilities

• Engage members to complete comprehensive needs assessment across medical, behavioral, and socioeconomic domains.
• Develop and implement person-centered care plans aligned with guidelines.
• Collaborate with care team and community resources to execute care plans.
