Job summary

Responsibilities include comprehensive needs assessments, care planning, collaboration with care teams, self-management coaching, and care transition coordination.

Qualifications

• Current, unrestricted RN license in New Jersey.
• Minimum 2 years of clinical experience.
• Proficiency with MS Office (Word, Excel, Outlook).
• Ability to travel up to 80% within assigned territory.

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing medical, behavioral, and socioeconomic needs.
• Collaborate with internal care teams, providers, and community resources to implement care plans.
• Provide education and coaching to support member self-management of care needs.
• Coordinate Care Transitions after ER visits, hospitalizations, or SNF admissions.
• Advocate for members and families to ensure needs are represented and supported.
