Job summary

UnitedHealthcare is seeking an RN Clinical Coordinator to manage a panel of members with complex medical and behavioral needs. The role focuses on in-home and field-based care in Mercer County and surrounding areas in New Jersey.

Responsibilities include comprehensive needs assessment, care planning, member education, and care transitions. Travel up to 80% within the territory is required, with strong collaboration across care teams and community partners.

Qualifications

• Current, unrestricted RN license in New Jersey.
• 2+ years of clinical experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel up to 80% within assigned territory.

Responsibilities

• Engage members in person or telephonically to assess medical, behavioral, functional, cultural and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, social determinants, and medication management.
• Collaborate with internal care team, providers and community resources to implement care plans.
