Job summary

UnitedHealthcare is seeking an RN Clinical Coordinator to manage a panel of members with complex medical and behavioral needs in Middlesex and surrounding counties, NJ. This field-based role emphasizes person-centered care and collaboration with the care team.

You will conduct needs assessments, develop care plans, coordinate with providers, and support discharge planning with travel up to 80% across the assigned territory.

Qualifications

• Current, unrestricted RN license in New Jersey.
• 2+ years of clinical experience.
• 1+ years of MS Office experience.
• Ability to travel up to 80% within assigned territory.

Responsibilities

• Engage members to complete comprehensive needs assessments (medical, behavioral, functional, cultural, socioeconomic).
• Develop and implement person-centered care plans addressing chronic conditions and social determinants of health.
• Collaborate with internal care teams, providers, and community resources to execute care plans.
• Provide education and coaching to support self-management and lifestyle changes.
• Support discharge planning and coordinate care transitions after ER visits or SNF admissions.
• Advocate for members to ensure their needs are represented by the care team.
