Job summary

Approximately 80% of time is spent in the field visiting members in their homes or long-term care settings, with travel to Mercer and surrounding counties, NJ.

Qualifications

• Current, unrestricted RN license in New Jersey.
• 2+ years of clinical experience.
• Access to reliable transportation with ability to travel up to 80% of time within territory.

Responsibilities

• Engage members face-to-face and/or telephonically to complete comprehensive needs assessments.
• Develop and implement person-centered care plans addressing medical, behavioral and social needs.
• Collaborate with care teams, providers and community resources to execute care plans.
