Job summary

Oak Street Health in Fayetteville, NC seeks a Registered Nurse to coordinate care and deliver high-quality nursing services within our primary care team.

You will work with a multidisciplinary team to provide triage, medication management, patient education and care coordination while maintaining HIPAA compliance and compassionate, patient‑centered care. This role emphasizes equity of care and strong teamwork to improve outcomes for our patients.

Qualifications

• Active RN licensure in North Carolina (NC)
• Current CPR and/or BLS certification
• EMR experience in a clinic setting
• Experience in a clinic setting with hands-on patient care and procedures
• HIPAA compliance and patient confidentiality
• Strong communication and assessment skills
• Ability to work independently and prioritize multiple tasks
• Collaborative attitude to work with an interdisciplinary team
• Able to lift up to 20 lbs and stand for long periods
• US work authorization

Responsibilities

• Provide competent nursing care in a safe, consistent manner
• Respond to telephonic requests and be reliable for patient needs
• Provide triage and symptom management for patients
• Utilize protocols for medication management and prior authorizations
• Review patient medications and update records as needed
• Educate patients and provide direct care for chronic conditions
• Collaborate on care plans and delegate tasks as appropriate
• Build relationships with patients and caregivers to support preventive care
• Create a welcoming environment with equity of care
• Participate in care team meetings and daily huddles
• Deliver exceptional patient experience with responsive care
• Perform point of care testing and phlebotomy as needed
• Other duties as assigned
