Job summary

Oak Street Health in Fayetteville, NC is hiring a Registered Nurse to build strong relationships with primary care patients, coordinate their care, and deliver a seamless, compassionate clinical experience.

You will provide competent nursing care, follow standardized care pathways, educate patients and caregivers, participate in daily huddles, and collaborate with the care team to improve outcomes and equity.

Qualifications

• Active RN licensure in North Carolina.
• CPR and/or BLS certification.
• Experience with Electronic Medical Records (EMR).
• Clinic setting experience with hands-on patient care.
• Adherence to HIPAA regulations and privacy.
• Strong communication and assessment skills.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Provide competent nursing care in a safe and consistent manner.
• Respond to telephonic requests and be reliable for patients.
• Provide triage and symptom management for on-site or remote patients.
• Use standardized protocols for medication management and refills.
• Review patient medications and update as needed.
• Educate patients and provide direct care, especially for chronic conditions.
• Collaborate with care team and delegate tasks as appropriate.
• Build relationships with patients and caregivers to support preventative care.
• Create a welcoming environment and focus on equitable care.
• Participate in care team meetings and daily huddles.
• Deliver an exceptional patient experience through service and respect.
