Job summary

Wellstar is seeking an RN Complex Care Coordinator to assess complex patient needs, coordinate care across the continuum, and engage with patients and families to ensure discharge planning is safe and patient-centered.

You will serve as an expert resource to the care team on clinical, psychosocial, and resource considerations, mentor new coordinators, and lead discharge planning for high-risk patients.

• Participates in interdisciplinary rounds and precepting of new staff.
