Job summary

Wellstar Health System is seeking a Care Coordinator RN to assess transitional care needs and coordinate across the care continuum. You will engage with patients and families to assure discharge planning meets goals and resources are used efficiently.

The role requires collaboration with physicians, the utilization review team, and interdisciplinary rounds to support care progression and timely discharge.

Qualifications

• Associates Nursing or Diploma in Nursing (RN) – Bachelor preferred.
• RN license required; multi-state Compact may be preferred.
• Minimum 1 year nursing experience in acute care.
• Strong communication, organization and problem solving.
• Knowledge of regulations and community resources.

Responsibilities

• Assesses chronic disease management and psychosocial needs.
• Plans discharge and coordinates across care teams.
• Facilitates patient/family education and conferences.
• Documents care progression and discharge plans.
• Collaborates with physicians, UM and care teams.
