Job summary

Wellstar Health System is seeking a Care Coordinator RN to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure care goals are met.

The CC RN plans efficiently for discharge, collaborates with the care team, and facilitates post-acute referrals to optimize care transitions. Minimum 1 year of acute-care nursing experience is required.

Qualifications

• Excellent written and verbal communication skills.
• Ability to collaborate with physicians, care team and patient/family.
• Understanding of regulatory requirements in healthcare.
• Ability to assess psychosocial and clinical factors to support discharge planning.
