Job summary

Wellstar Health System seeks an RN Care Coordinator to assess transitional care needs, coordinate services across the care continuum, support care progression, and help patients transition safely from the hospital. You will partner with physicians, utilization review nurses, and care teams to address needs and remove discharge barriers.

The role supports collaborative care planning and aims to improve patient outcomes with a focus on seamless transitions and comprehensive discharge planning.

Qualifications

• Active Registered Nurse license.
• At least 1 year of nursing experience in an acute care setting.
• Current Basic Life Support (BLS) certification.
