Job summary

Qualifications

• Minimum of an Associate’s or Bachelor’s Nursing degree.
• RN license and BLS certification required.
• At least 3 years healthcare experience, including 2 years in care coordination in an acute care setting.
• Strong communication and organizational skills.

Responsibilities

• Conduct comprehensive clinical and psychosocial assessments.
• Coordinate care across the continuum and facilitate timely discharges.
• Collaborate with interdisciplinary teams and mentor staff.
• Assess insurance, resources, and document interventions.
