Job summary

UnitedHealth Group seeks a Telephone Case Manager RN in Atlanta, GA to educate, advocate, and connect members with resources, delivering meaningful health outcomes in a flexible setting.

You will handle outbound/inbound calls to assess health status, identify barriers, provide self-management education, and coordinate referrals for continuous, quality care. BSN preferred; 10+ years in case management or related care settings; active RN license and compact license.

Qualifications

• 10+ years of experience in case management, hospital, acute care, or direct care settings.
• Current, unrestricted RN license in the state of residence; Compact RN license required.
• BSN preferred.

Responsibilities

• Make outbound calls and receive inbound calls to assess members' current health status.
• Identify gaps or barriers in treatment plans.
• Provide patient education to assist with self-management.
• Make referrals to outside sources.
• Provide a complete continuum of quality care through close communication with members.
• Support members with condition education, medication reviews, and connections to resources.
