Active RN licensure, ADN (with BSN preferred), and 5+ years of RN experience are required. Compassionate care delivery with regulatory compliance is essential, and there is consideration for a company-paid Compact RN license.

Qualifications

• Active, unrestricted RN license in state of residence.
• Willingness to obtain a Compact RN license (company-paid).
• Minimum 5 years of clinical RN experience.
• Associate Degree in Nursing (ADN).
• Ability to work within Monday-Friday business hours.

Responsibilities

• Conduct telephonic assessments and develop care plans.
• Identify health risks, barriers, and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to clinical programs.
• Apply clinical judgment to support members with complex conditions.
• Collaborate with multidisciplinary teams to achieve outcomes.
• Maintain compliance with regulatory requirements and company policies.
